Position Highlights
Full-time, outpatient primary care with 4- or 5-day work week options
Quickly build your practice with an established patient base
Provide comprehensive care for patients of all ages, including acute and chronic disease management
Opportunity to perform screening colonoscopies
Ability to perform small office procedures (laceration repair, wart removal, I&D, etc.)
Shared after-hours call supported by a nurse triage system
Access to on-site and nearby services including: Outpatient lab, Pharmacy, Radiology, Physical and Occupational therapy
Opportunity to teach and mentor residents through the Family Medicine Residency Program at IU Health Ball Memorial Hospital
Strong team culture focused on quality, patient experience, and provider support
Community-based practice with the backing of a large, integrated health system

Role Expectations
Establish and manage a patient panel drawn from Hartford City and surrounding areas
Use measurable quality-based data to improve patient outcomes and support population health
Deliver evidence-based, compassionate, and equitable care

Ideal Candidate
MD or DO with board certification or eligibility in Family Medicine or Internal Medicine
Passion for rural health and community engagement

Hartford City is the county seat of Blackford County. Located in the northeast central portion of the state, the city of approximately 6,000 has a strong legacy of agriculture & manufacturing. The town square is a "living museum" and is listed on the National Register of Historic Places.
