Family Medicine Physician

A New York-based healthcare provider is seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in New York County and Bronx County.

About the Opportunity:

• Schedule: Weekdays and weekends
• Hours: Minimum 9 hours/week
• Setting: Community-based

Responsibilities:

• Reviewing and recommending on consumer assessments
• Maintaining knowledge of regulatory changes and clinical quality
• Validating assessment outcomes using clinical skills
• Collaborating on quality assurance and improvement initiatives
• Supporting program goals through education and communication

Qualifications:

• 2+ years of experience as an APP
• Master's Degree (NP or PA Program)
• Active New York State License
• Active Medicaid Number
• BLS Certification
• Computer literacy and tech navigation
• Experience with telehealth assessments
• Strong written and oral communication
• Interpersonal skills and adaptability

Desired Skills:

• Experience with Adult/Geriatric populations
• Knowledge of Long-Term Care programs
