Registered Nurse (Monday to Friday PM & ND)

Responsibilities will include:

Coordinate and oversee the provision of high-quality clinical care for residents

Supervise and mentor a team of Enrolled Nurses and Personal Care Workers.

Create and review care plans, medication administration, wound management.

Liaise with allied health professionals, GPs, and families to ensure holistic care.

Monitor and evaluate clinical outcomes, making adjustments as needed.

Ensure compliance with all relevant policies, procedures, and regulations.

Actively promote, participate and evaluate continuous quality improvement in clinical services.

Maintain accurate, legible records of clinical care provided in all client interactions

Excellent assessment, communication, and time management skills.

Ability to work efficiently, autonomously, and respectfully within the team.

Enrolled Nurse (AM & PM)

Responsibilities will include:

Personal Care: Assisting residents with activities of daily living (ADLs) such as showering, dressing, toileting, and grooming.

Medication Administration: Administering medications as prescribed by the physician and ensuring proper documentation of medication records.

Health Monitoring: Observing and recording residents' vital signs, symptoms, and any changes in their health condition.

Wound Care: Providing basic wound care and dressing changes for residents with minor wounds.

Assessment and Reporting: Conducting health assessments and reporting any changes in residents' health to the supervising RN or healthcare team and on Autumncare.Deliver, participate and assist in the assessment, formulation and review of resident care plans.

PCA (Monday to Friday AM, PM & ND)

Cro Care is seeking compassionate and dedicated Carers to join our supportive team to work variety of AM, PM & ND shifts, Monday to Friday available with us. As a Carer you will be supported by the clinical team and will be responsible for providing person centred care to residents. You will support the clinical and lifestyle teams to allow residents to live the lives they value.

Responsibilities will include:

Assist with personal hygiene tasks such as bathing, grooming, and dressing.

Support with mobility and transferring individuals safely.

Monitor clients and report any changes in health status.

Provide emotional support and companionship to individuals.

Maintain cleanliness and organisation in the individual's living environment

Following the care plan, work to a planned schedule under the direction of a registered nurse.

Record consumer outcomes into the appropriate Cro Care records.

Attend regular scheduled training to maintain your level of skill.

About you

Your skills and experience will include;

For Registered Nurses:

Current registration with the Nursing and Midwifery Board of Australia and Australian Health Practitioner Regulation Agency (AHPRA).

Substantial experience in aged care, including a strong background in clinical leadership and care coordination

Exceptional clinical assessment and decision-making skills

Demonstrated ability to lead and motivate a team of healthcare professionals

Excellent communication and interpersonal skills, with a focus on collaborative, person-centred care

Strong understanding of aged care regulations, accreditation requirements, and best practices

Demonstrated clinical skills according to level of experience using a problem solving approach to nursing practice within a primary care corrections setting.

Demonstrated effective communication and interpersonal skills across a wide range of cultures.

Demonstrated knowledge and application of continuous Quality Improvement process.

Demonstrated ability to work as a team member and leader.

For Enrolled Nurses:

Current registration with the Nursing and Midwifery Board of Australia and Australian Health Practitioner Regulation Agency (AHPRA).

A strong commitment to high quality care.

Minimum of 12 months previous experience in Residential Aged Care or sub-acute nursing.

Highly developed communication and interpersonal skills.

The ability to work as part of a diverse team.

For Personal Care Assistants:

Certificate III in Aged Care or Individual Support.

Minimum 6 months experience in Aged Care

Nursing Students who have completed 120 hours of nursing placements.

2 professional referees.

Valid Police Check.

Please Note:

We are looking for staff who can commit to, at minimum, 3 days per week. As part of the application process, you will be required to complete pre-employment checks which may include, but is not limited to reference checks, work rights, criminal history check, NDIS/WWCC/WWVP checks, and a health evaluation to ensure suitability for the role.

Must have Aged Care experience to be considered .

Set on a large site with beautiful, landscaped gardens located in Geelong, with an allocation of 100 beds, including the ability to accommodate 28 residents in our secure Memory Support Unit.
