We have an in-house clinical team responsible for delivering high-quality, person-centred clinical care to our participants in their own homes.

This is a Permanent Full-time role Monday - Friday working in the community. Working hours are between 7.30am and 3.30pm or 8am - 4pm. Join a dedicated team who provide clinical services, conduct comprehensive nursing assessments and ongoing clinical reviews.

You will develop, implement and evaluate individual care plans; administer medications and complex clinical interventions in accordance with legislation and scope of practice. You will deliver a range of services and focus on building strong, meaningful relationships with our participants, empowering them to remain independent at home and stay connected. Minimum 2 years post graduate clinical experience. Required to possess a broad range of clinical skills and competencies including but not limited to, the areas of medication management, continence assessments, wound management, and chronic disease awareness & preventative health care Strong clinical assessment, decision‑making and documentation skills. Excellent interpersonal and communication skills.

Ability to work autonomously and manage time effectively. Current Drivers Licence Demonstrated understanding of person-centred care and a willingness to work within Southcare’s Purpose & Values. National Police Clearance in line with the requirements of the Aged Care Act.
